Job Requirements:

    Degree/Education: Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted license to practice as a Registered Nurse.

  • Experience: A minimum of 3 years (ADN) or 2 years (BSN) full-time experience in clinical case management with diverse populations. Familiarity with the military healthcare system and telehealth experience is desirable. Work experience with Tri-care and Military Health System Genesis system preferred.

  • Certification 1: Certified Registered Nurse Care Manager by Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC). Certification eligible with 3 years of experience (ADN) or 2 years of experience (BSN) in full-time clinical case management.

  • Certification 2: Life Support Certification: Maintain current certification in American Heart Association Basic Life Support (BLS). Additional certifications such as ACLS, PALS, and NRC as required by AFI 44-102 or equivalent.

  1. Comprehensive Case Management: Provide expert case management for military members, veterans, and their families across various healthcare settings, focusing on acute, chronic, and complex health issues.

  2. Patient Advocacy: Serve as a dedicated advocate for military members, ensuring they receive the highest standard of care, while addressing their physical, emotional, and psychosocial needs.

  3. Collaborative Care Coordination: Work seamlessly with interdisciplinary healthcare teams to coordinate and integrate care across all levels and sites, facilitating a holistic approach to patient well-being.

  4. Military Community Engagement: Engage with the military community, fostering relationships with service members and their families, and contributing to the unique healthcare challenges faced by those serving in the armed forces.

  5. Innovative Healthcare Solutions: Contribute to the development and implementation of innovative healthcare solutions, including telehealth initiatives, to enhance accessibility and quality of care for military members.
